Your Child's Appointment: What to Bring with You
The Yale Hearing and Balance Center asks that you bring the following items with you for your child's appointment:
- The child's current hearing aids
- List of all current medications (include the name of the medication and the dosage)
- Medical records that relate to your child's hearing
- Your current health insurance identification card and any referral/authorization forms that may be required by your insurance company
We will do our best to see your child promptly at his/her scheduled appointment time. Because of the nature of our practices, we sometimes have emergencies that can cause a delay. We appreciate your understanding when other patients require extra time, and we promise to give you the same undivided attention when you are seen.
What to Expect the Day of Your Visit
The Yale Hearing and Balance Center is part of a teaching hospital. During your appointment, you and your audiologist may be joined by a graduate student clinician. Please know that this increases the quality of the care that you receive at Yale.
